For your profile on LJ? Find the icon you want, right click on it and choose "Save image..." and choose where you want to save it to. I usually save it to my desktop so I can find it easier.

Then go here: http://www.livejournal.com/editpics.bml and to the left in the blue box click "browse," choose the icon you saved, and click open.

In the keywords section you can name the icon something like "Britney Spears - Hold It Against Me" or name it anything you want.

In the comment section, you type what I show in the photo here:
